Professional Fees

Our optometric and medical services are provided based on the definitions found in the American Medical Association’s Current Procedural Terminology (CPT). The CPT Codebook describes how to document and report eye examinations, diagnostic tests, contact lens exam services, and surgical procedures.

Medical Necessity

Professional services are provided at different levels of intensity according to the patient’s medical need and the intensity of an eye examination is based on the following:

  • Patient’s history
  • Nature of the presenting problem
  • Eye doctor’s clinical judgment

Comprehensive Eye Examination

  • New patient – $150
  • Established patient – $130

Intermediate Eye Examination

  • New patient – $90
  • Established patient – $90

Level 1 Evaluation & Management Service

  • New patient – $45
  • Established patient – $25

Level 2 Evaluation & Management Service

  • New patient – $75
  • Established patient – $45

Level 3 Evaluation & Management Service

  • New patient – $110
  • Established patient – $75

Level 4 Evaluation & Management Service

  • New patient – $165
  • Established patient – $110

Level 5 Evaluation & Management Service

  • New patient – $200
  • Established patient – $140
